ThermiVa and O-Shot procedures are not covered by insurance. Discuss costs with your provider if you are interested in these services.

We do not participate with any Medicaid plans. Our practice requires that all patients keep a valid credit card on file for our no-show, late cancellation policy.

All patients must pay at time of service. Co-pays, deductibles and cash payments are due at the time of the visit.
